Qualtrics Integration

I'm working on a guide that integrates Qualtrics! We created a qualtrics XM survey, and put the anonymous link into our code block. I've been following these instructions on how to create a guide. However, every time I try to run the guide I get error messages.

Does anyone have input as to why this isn't working? Thanks!
